The Patient Form (?page=patient-form) is used both to register a brand-new patient and to edit an existing one. It captures demographics, contact info, emergency contact, clinical assignment, insurance, ABHA (Ayushman Bharat Digital Mission) details, and patient portal access — all in one form.
Step-by-step
- Go to Patients → Add Patient.
- Fill in Personal details: First/Last name (required), date of birth, gender, phone, email, Telegram chat ID (for notifications).
- Fill in Demographics: blood group, marital status, occupation, education.
- Fill in Address: address, city, state.
- Fill in Emergency contact: name, relationship, phone.
- Set Referral source and Status (e.g., active/critical — marking a patient critical requires a documented reason).
- Assign a doctor from the dropdown.
- Optionally add insurance provider/number.
- Optionally link an ABHA number/address for national health record integration.
- Optionally enable patient portal access — set a portal email and password (or leave password blank on edit to keep it unchanged).
- Add any general notes.
- Click Save Patient, or Save & Start AI Intake to immediately begin an AI-assisted intake session for this patient.
Marking a patient critical
Changing status to "critical" requires entering a reason; the system stamps who marked it and when. If the reason is only edited (not the critical flag), the original marker/timestamp is preserved. Un-marking critical clears the stored reason.
