Registering a New Patient

The Patient Form (?page=patient-form) is used both to register a brand-new patient and to edit an existing one. It captures demographics, contact info, emergency contact, clinical assignment, insurance, ABHA (Ayushman Bharat Digital Mission) details, and patient portal access — all in one form.

Step-by-step

  1. Go to Patients → Add Patient.
  2. Fill in Personal details: First/Last name (required), date of birth, gender, phone, email, Telegram chat ID (for notifications).
  3. Fill in Demographics: blood group, marital status, occupation, education.
  4. Fill in Address: address, city, state.
  5. Fill in Emergency contact: name, relationship, phone.
  6. Set Referral source and Status (e.g., active/critical — marking a patient critical requires a documented reason).
  7. Assign a doctor from the dropdown.
  8. Optionally add insurance provider/number.
  9. Optionally link an ABHA number/address for national health record integration.
  10. Optionally enable patient portal access — set a portal email and password (or leave password blank on edit to keep it unchanged).
  11. Add any general notes.
  12. Click Save Patient, or Save & Start AI Intake to immediately begin an AI-assisted intake session for this patient.

Marking a patient critical

Changing status to "critical" requires entering a reason; the system stamps who marked it and when. If the reason is only edited (not the critical flag), the original marker/timestamp is preserved. Un-marking critical clears the stored reason.

Common issues & fixes

the form blocks saving without these.
make sure a portal email was set and the password (min. 8 characters) was entered; on edit, leaving the password blank keeps the existing one.
entering a raw chat ID manually overrides any previously recorded @username link, since they may no longer match the same account.
search the registry by phone/email before creating a new record.
