The Seclusion & Restraint Register (ipd/pages/seclusion_restraint.php) is a mandatory episode log required under India's Mental Healthcare Act (MHCA) Section 97, covering the use of seclusion or restraint on an inpatient — including ongoing monitoring during the episode and a mandatory debrief afterward. Requires ipd_nursing_charts.

Episode reference numbering

Each episode is automatically assigned a unique reference in the format PREFIX-YEAR-NNNN (e.g., SR-2026-0001), configurable via the clinic's episode prefix setting, incrementing per year.

Step-by-step

  1. Go to Seclusion & Restraint Register for the admission.
  2. Log a new episode: reason, type (seclusion/restraint/both), start time, and clinical justification.
  3. Record ongoing monitoring entries throughout the episode as required (vitals, behavior checks at set intervals).
  4. Record the end time once the episode concludes.
  5. Complete the mandatory debrief — review with the patient and staff, lessons learned, and any care plan changes.
