Incident Reports (ipd/pages/incidents.php) logs safety events on the ward — falls, self-harm episodes, behavioral incidents, and similar occurrences — for a specific admission. Requires ipd_incidents. (This is distinct from the general Security Incidents register, which covers data-breach/privacy events under DPDPA.)

Step-by-step

Go to Incident Reports for the admission.
Log the incident: type (fall, self-harm, behavioral, other), date/time, description, and immediate actions taken.
Note any injuries and follow-up care provided.
Save — the incident becomes part of the admission's safety record and may inform care plan or observation-level changes.
