MHCA Documents & Forms (Statutory Forms A–J)

This is a set of three connected pages for handling Mental Healthcare Act 2017 statutory compliance:

  • MHCA Documents (ipd/pages/mhca_documents.php) — the compliance dashboard for an admission, showing which required MHCA documents have been completed.
  • MHCA Forms Library (ipd/pages/mhca_forms.php) — the library of official statutory Forms A through J (e.g., admission consent, nominated representative, advance directives, and related forms) available to generate for an admission.
  • MHCA E-Sign (ipd/pages/mhca_esign.php) — a standalone, unauthenticated page reached via a secure single-use token link, letting a patient, attendant, or family member remotely sign a required form without needing a staff login.

All three (except e-sign, by design) require the ipd_mhca_documents permission.

Step-by-step: generating and completing a form

  1. Go to MHCA Documents for the admission to see the compliance checklist.
  2. Open MHCA Forms Library, choose the applicable form (A–J), and fill in the required fields.
  3. If the form needs a signature from someone without system access (patient/family), generate a secure e-sign link and share it — they open it via the link, no login required, and sign there.
  4. Once signed, the completed form appears back on the MHCA Documents checklist and can be printed via the dedicated print view.

Common issues & fixes

requires ipd_mhca_documents; this is a separate permission from general progress notes or nursing charts.
these links are single-purpose and secure; if expired or already used, generate a new one rather than reusing an old link.
double check all required fields in the Forms Library entry were completed before printing, since incomplete statutory forms may not hold up to compliance review.
