Nursing Charts (ipd/pages/nursing_charts.php) is where nurses record shift-by-shift observations, vitals, and behavioral monitoring for an inpatient — the core nursing documentation for the admission. Requires ipd_nursing_charts.

Step-by-step

  1. Go to IPD → Nursing Charts for the admission.
  2. Select the current shift (morning/afternoon/evening/night).
  3. Record vitals, general observations, behavior/mental state notes, and any events during the shift.
  4. Save — entries accumulate into a chronological nursing record reviewable by the care team.

Common issues & fixes

requires ipd_nursing_charts.
chart as soon as possible after each shift; late entries should be clearly noted as such rather than backdated silently.
nursing chart vitals are specific to the IPD shift-charting workflow; they may overlap conceptually with the OPD Vitals feature but are tracked separately for inpatient continuity.
