These 17 forms reproduce the text prescribed in the Mental Healthcare (Rights of Persons with Mental Illness) Rules, 2018 and the Mental Healthcare (Central Mental Health Authority) Regulations, 2020, as published in the Gazette of India. Some states additionally require their own State Mental Health Authority registration form, which is not included here as it varies by state; see our MHCA compliance checklist and licensing guide for state-specific detail. Rules can be amended; verify current wording against the official Gazette before relying on a generated form for a legal proceeding.
Seventeen forms across four groups: the advance directive form for making, amending, revoking or cancelling a directive; six medical records forms, including the request form and templates for outpatient records, inpatient records, psychological assessment reports and therapy session notes; six admission and discharge forms covering independent admission, minor admission, high-support admission under Sections 89 and 90, and discharge; two leave and police intimation forms; and two establishment forms for permanent registration and objections to registration, plus the physical restraint monitoring form required under Section 97 of the Act.
