NDPS Register Template

Printable register formats matching the fields a de-addiction centre or pharmacy is expected to maintain for controlled substances, ready to fill by hand or adapt into a spreadsheet.

Two different regimes, don't mix them up. Buprenorphine, the medicine most Indian de-addiction centres actually dispense, is not on the central "Essential Narcotic Drugs" list, so it is governed by your state's own de-addiction centre rules rather than the central Form 3D/3E format. Buprenorphine stock & dispensing register

Modelled on the register structure used under state de-addiction centre rules such as Punjab's (Annexures C and D of the private centre SOP). Field names in your state's actual notified format may vary slightly, always cross-check against your own license conditions.

1. Central stock / consumable register (one section per medicine and strength)

Keep a separate register, or clearly separate pages, for each medicine and strength (e.g. BNX 2mg/0.5mg vs BNX 8mg/2mg). Stock should be kept in a locked almirah with nurse-only key access, and reconciled against this register regularly by the centre's nodal officer.

2. Patient-wise dispensing register (one page per registered patient)

Every patient should carry a unique registration or UID number, ideally linked to Aadhaar or mobile number per most state rules, so the same patient cannot draw medication from multiple centres.

Essential Narcotic Drugs register, central Form 3D / 3E

For morphine, codeine, oxycodone, hydrocodone, fentanyl and methadone, the drugs classified as Essential Narcotic Drugs (ENDs) under the 2014 NDPS Act amendment and 2015 NDPS Rules. Records must be preserved for a minimum of two years from the date of the last entry and produced to authorised officers on demand.

Form 3H (main drug storage room record) follows the same structure as Form 3D, applied at the central pharmacy or store level rather than the ward level. An annual return is filed using Form 3-I.
